SAFETY ALERT/ACTION NOTE KMG BOOSTER bomber

There have been at least two serious accidents in the last 24 months involving these
rides. Both have resulted in serious physical injuries to the operator and both appear
to have the same root cause. Examples of the KMG Booster, one set up to run and
another ready for transport can be seen at Figs 1 and 2.

SAFETY ALERT/ACTION NOTE CRAZY FROG TYPE RIDES

This alert results from the now closed investigation into the Crazy Frog accident on
Blackpool Central Pier on 29 July 2009. The investigation involved HSE Specialist
Mechanical Inspectors and Specialists from the Health and Safety Laboratory (HSL).
The accident occurred when a radial arm collapsed whilst the ride was running. Two
riders were in the car at the time but there were no serious injuries. A photo of the
ride with the broken arm in situ is at Fig 1.

SAFETY ALERT/ACTION NOTE KOLMAX PLUS MIAMI TRIP

On 12 June 2010 at Halifax a young, disabled person was injured when they were
ejected from the right hand end seat a 20 seat Kolmax Plus Maimi Trip ride. The ride
had been manufactured in the Czech Republic in 2004. See Fig 1.

“Fairmatt” Type Miami Cracking

We have received notification of defects on a Miami Trip rides manufactured by Fairmatt Ltd,
Eurofab Ltd, Nottingham (UK) Ltd &FGL. Cracks have been found at the junction of the spur wheel
mounting plate and the main arm box-section as shown in figure 1.
